A teacher questionnaire can make a huge difference when giving birthday, Christmas or Teacher Appreciation gifts. I mean, we’ve all gotten gifts we don’t actually want, right?!
Not that YOU give bad gifts or anything…
But wouldn’t it be better (and easier) to give your child’s teacher something he/she actually wanted? Or better yet, needed?!
A Printable Teacher Questionnaire to the Rescue
When I was in elementary school, I dreamt of becoming a teacher simply so I would be able to grade papers and create worksheets.
Turns out, I had no interest in the actual teaching part. 😅
Long story short, I never became a teacher, BUT my love for formatting documents never faded with time. I seriously love it. 🤓
Now, after becoming an accidental room mom, I’m putting this weird hobby to work. Room moms deserve pretty + functional printables, especially when first getting to know their child’s teacher.
With this in mind, we’ve put together a free, full-color PDF questionnaire for you to break the ice with your child’s teacher while possibly earning a few brownie points in the process. 😉
You can print these questions for the teacher and send them to school with your kiddo. Or you can e-mail it to the teacher. Even easier!
If you’re looking to save some ink, there’s also a printer-friendly version of the form!
This printable PDF is completely free inside the Resource Library. But if you’re wanting to customize the teacher questionnaire, we recently added EDITABLE versions of this form in our editable questionnaire bundle! Check it out:
Sometimes it’s nice to customize the questions and tailor them to the teacher. For example, if you know your child’s new teacher LOVES cats, but you’re not sure what kind or how many cats she has, you can create a line just for that!
What to Do with Your Completed Teacher Questionnaire
Once the teacher returns the completed questionnaire, you can attach it to your room mom intro letter to parents. I like to include this with my introduction letter because it provides VALUE to the other parents that they can use throughout the year.
As s room parent, I always want to be as helpful as possible. Including a list of all the best gift ideas for the teacher is a great way to start off!
Oh, and if you don’t have an updated list of parent emails, use this template to get them fast!
- Is this template too girly for the male teacher? Check out this All About You printable.
- No printer? No problem. Use this email teacher questionnaire template to streamline your teacher survey OR use the fillable teacher favorites questionnaire version!
- Looking for a more customizable teacher Q&A? Use this editable template to make your own unique form.
- Make sure to send this questionnaire to the class parents along with your Room Mom introduction letter!
I hope this teacher questionnaire helped you get to know your kiddo’s teacher this year! Once he/she fills in those blanks, you’ve got an instant teacher gift cheat sheet. What’s better than that?! 😊
Leave a comment if you have any suggestions for items I should add to my next form. Or, if you customized your own printable, what did you add to the list?? I’d seriously love to know!
Have a great school year,
